For The Long Term Care Facility

Clinical pharmacist consulting

Health One Consultant Pharmacists are specially trained to help you effectively manage outcomes and costs.

Your Health One Consultant Pharmacist will provide regularly scheduled drug regimen reviews to assess compliance with all state and federal regulations, pharmaceutical standards, and standards set by State Boards of Pharmacy, JCAHO, and CARF. In addition, your Consultant Pharmacist will identify high-cost and high-risk therapies within your patient population and develop protocols to effectively manage these therapies.

Upon completion of each review, the Consultant Pharmacist will provide a detailed findings report and will monitor the facility’s progress on recommendations.

Other clinical services provided by Health One include:

  • Drug Utilization Reviews and Quality Assessment Audits to assist in maximizing the benefits of drug therapy, minimizing adverse drug events, and managing drug cost for the facility.

  • Scheduled med pass audits at each facility.

  • Periodic review of procurement, administration, storage, and documentation of drug therapies.

  • Instruction in proper medication administration

  • Nursing orientation support

  • A comprehensive Pharmacy Policy and Procedure Manual, to support procurement, storage, administration, monitoring, and documentation protocols.
